What’s the difference between a Care Partner and a Sandwych Care Manager?

  • A Care Partner is a loved one that you designate as knowing most of your health information in a non-professional capacity -- a spouse, child, parent, close friend. This could also be a court appointed advocate or an advocate of your choosing.

  • A Sandwych Care Manager is a trained professional who uses Sandwych to help guide your care, keep everyone connected, and make sure nothing falls through the cracks.

Think of Care Partners as the people who provide care, and Care Managers as the people who help coordinate it.

What do you mean by a ‘provider’?

  • A provider is any licensed medical or behavioral health professional who gives you care — like doctors, nurses, therapists, or home health agencies.


What is a Local Resource?

  • A stand-alone entity with a valid EIN serving within one of the seven components of human health: Environmental, Physical, Emotional, Intellectual, Social, Spiritual, and Financial.

    A Local Resource is one that can fill in care gaps for SandwYch users or anyone else looking for assistance. There are 3 levels of support a Local Resource can provide through SandwYch, which are detailed here.


TECHNICAL QUESTIONS

Do I have to pay for SandwYch?

  • If you are referred to SandwYch through a provider, a health plan, or a Verified Local Resource, or SandwYch Care Manager you will be able to enjoy SandwYch for FREE with a code they’ll provide.

    If you are not able to obtain a referral, you can contact us about a care advocate.
